2020 Board of Directors Nomination Form

Please ensure you have the following photo/image files to include in your submission:

  • Nominee’s name, signature, date and written statement of Acceptance of Nomination (see sample photo below)
  • Each Nominator’s (minimum three) name, signature, date and statement of nominating the nominee (see sample photo below)

If you require assistance, please call the Union Office at 780-448-8900 between 8:00 a.m. and 4:30 p.m., Monday to Friday.

  • Select the Board Position for which the nominee is being nominated for. All positions are for 2-year terms: January 1, 2021 to December 31, 2022.
  • Enter the first and last name of the Active Member in good standing who is being nominated into the above Board Position.
    *For the nomination of a Contractual Unit Director, the nominee and nominators must be members of the Contractual Unit in question.
  • Enter the first and last name of at least three Nominators who are Active Members in good standing. Select the plus icon (+) to add more rows.
    *For the nomination of a Contractual Unit Director, the nominee and nominators must be members of the Contractual Unit in question.
  • Attach required photos:
    Nominee name, signature, date and written statement of Acceptance of Nomination of [Board Position]
    Each Nominator’s (minimum three) name, signature, date and written statement of nominating [Nominee’s name] for [Board Position]
